Both want the same words — “longevity,” “anti-aging,” “biological age.” But they’re different categories solving different problems. The first is entertainment with a wellness wrapper. The second is medicine that happens to be using newer tools.
This piece is for patients trying to figure out which side actually helps them, written from the clinical side by a DAOM running multi-modality longevity protocols at the Tustin clinic.
Why the spectacle wing gets the attention
Three structural reasons:
- It’s filmable. A 47-year-old man taking 110 supplements at 5 AM, getting his face skin replaced, undergoing plasma exchange with his teenage son — that’s documentary content. Measured improvement in HRV over 18 months is not.

Why clinical longevity stays quiet
Three structural reasons:
- It’s measured. Reputable clinical longevity practitioners publish hedged ranges, not certainties. That doesn’t drive virality.
- It’s personalized. What worked for the 47-year-old male with high-T baseline doesn’t work for the 51-year-old perimenopausal woman with sluggish thyroid. Personalization is the opposite of mass-distributable content.
- It’s slow. Real biological-age shifts take 12-36 months of consistent intervention. The patient doesn’t see a daily Instagram update; they see a quarterly DEXA scan or a monthly bloodwork trend.
Quiet doesn’t mean small. The clinical longevity wing is where most actual progress lives.
Where Bryan Johnson is probably right
Crediting first:
- Cadence matters. Doing health protocols sporadically gets you sporadic results. Bryan’s commitment to consistent measurement is the right structural choice — even if the specific protocol stack isn’t.
- Sleep is foundational. His ruthless prioritization of sleep efficiency (>95% target) is correct biology. Most patients don’t take this seriously enough.
- Self-experimentation has a place. Running n=1 protocols on yourself with real biomarkers is legitimate science when done with rigor and humility.
- Cosmetic ≠ unnecessary. The skin and hair work get mocked, but biological skin age does track with systemic aging markers. He’s just doing it loudly.
Where it stops generalizing
- N=1 doesn’t scale. What works for one 47-year-old male with elite-tier baseline biology and a $2M/year health budget tells you almost nothing about what will work for you.
- Supplement burden has a cost. Taking 100+ supplements creates GI burden, methylation chaos, and unmeasured cross-reactions. Most patients can’t tolerate his stack even if they could afford it.
- Time and money are real constraints. Bryan optimizes for not dying. Most patients have to optimize for “not dying and working a job and raising kids and maintaining relationships.” Protocol fit matters.

Enhanced Games positions itself as “Olympics with allowed performance enhancement.” The “infomercial” reception (per industry coverage as of W22) is the right read — the actual structural problem isn’t that doping happened, it’s that the entire framing collapses the line between athletic performance and longevity science. They are not the same field. Enhanced Games is sports entertainment. It’s not a longevity protocol, even though it borrows the vocabulary.
If a patient asks me whether Enhanced Games athletes are “doing longevity,” the honest answer is no. They’re doing pharmacological performance enhancement for a specific event, often with documented health costs downstream. That’s not the same thing as compressing morbidity or extending healthspan.
What clinical longevity actually does
